NHS Continuing Healthcare
What is NHS Continuing Healthcare?
If a person you support has a serious health condition or needs specialist support from a nurse, you might want to find out about NHS funded care.
This is called Continuing Health Care, or CHC.
NHS CHC is care and support that is provided to an individual aged 18 or over. In other words, the NHS will meet any health and social care needs that have arisen as a result of disability, accident or illness.
